Patient-centered care, is a practice in which health caregivers anticipate the discomforts or fears of patients when receiving care and take the responsibility for comforting and reassuring their patients at every moment, making sure that the patient is at the center of everything they do. This is very necessary because being admitted in hospitals frightens very many people and very few want to be in the hospital. In addition, an increasing body of evidence has connected patient centered care practices with improvements in a variety of health conditions, increased adherence, decreased healthcare utilization, better ratings of care and improved quality of care (Fix et al.2018). Despite the fact that patient care is constantly evolving, due to technological advancements, the expectations of most patients are not.
Patients always expect a high-quality experience, therefore the culture of patient-centeredness is a must “need to have. The importance of a patient-centered care model.
In a patient-centered care model, it is paramount for patients to always be in complete control when it comes to making decisions about their own care and treatment. Clinicians are there to inform, advice and support, but it is in the authority of the patient to determine what course of action they will take. Therefore, a patient needs to fully understand the procedures and treatments they will undergo, and clinicians must aim to make them as knowledgeable and comfortable as possible, especially when treating them for a complicated or painful illness or injury.
The value of evidence-based practice
Evidence-based practice is a widely used problem-solving approach in the clinical setting, but it’s also crucial to delivering patient-centered care. This integrates clinical expertise with the latest and best research evidence, along with known patient values, in order to deliver the best possible patient care. One of the most important elements of evidence-based practice in nursing is taking into account the needs and wants of the individual patient. We need to understand what is most important to our patients; what are their preferences when it comes to treatment options, and how do they define quality of life? All of these things come together to make up the definition of evidence-based healthcare. In the end, it is important to never lose sight of the fact that all patients deserve to be respected and must to be at the center of focus during every healthcare encounter. Patients expect nurses and other healthcare professionals to be knowledgeable, competent and confident, but also kind and understanding too. Nurses are ambassadors for their institutions, the profession and for their patients. Facilitators to patient-centered care included: 1) Leadership commitment. 2) Leadership support. 3) Training and education for patient-centered care (Balint, 1969).
Barriers to the implementation of patient-centered evidence-based care
Implementation of patient centered care faces several roadblocks and they are as follows: Time constraints as the patient centered care model takes more time to practice. For a patient to be able to make informed decisions about a particular treatment, the condition they are experiencing must be explained in simple and understandable terms. Lack of clarity on what patient centered care constitutes as made it way hard to practice it as well as explain to patients. Another barrier of patient centered care is the weakening of the professional power-The staff now experience professional status reduction and have to compromise in decision making as well as in autonomy (Balint, 1969).
Possible remedies to these challenges include:
Time constraint challenge can be remedied by improving patient efficiency though educating them about the patient centered care mode. Development of clinical guidelines that details all what patient centered care should achieve and it should be done. And lastly, developing innovative ways to deal with patients who have communication problems for example using picture symbols for patients with hearing problems.

In conclusion, in the end, it is important to never lose sight of the fact that all patients deserve to be respected and must to be at the center of focus during every healthcare encounter. Patients expect nurses and other healthcare professionals to be knowledgeable, competent and confident, but also kind and understanding too. Nurses are ambassadors for their institutions, the profession and for their patients.

The Institute of Medicine defines patient-centered care as providing care that is respectful of and responsive to individual patient preferences, needs and values, and ensuring that patient values guide all clinical decisions. Patient centered care is geared to meeting the patient’s needs by being respectful towards that patient’s values, culture and personal beliefs. Most patients’ ideals and beliefs stem from how they were raised and family beliefs. The patient perception of healthcare and medical needs can also be influenced by spiritual, religious, social as well as cultural norms. In some instances people are raised to have specific values and beliefs, but as they grow and evolve, these values and beliefs do change.
Barriers to patient centered care
Some realistic barriers include time constraints, lack of communication upon discharge and patients not feeling prepared to discharge. There are programs put in place to help healthcare institutions implement processes to help them provide more patient centered care. I currently do contract nursing and the one thing that obviously needs to happen in an effort to alleviate barriers is retaining employees. In order to provide enough time to take care of patients and provide patient centered care, there needs to be sufficient staff.. Every facility I do contract work for is severely understaffed. “Although nursing staff shortage is a significant barrier to patient-centered care and communication that healthcare institutions and managers must know, some healthcare scholars have critique nurses’ complaints of time limitation.”(Kwame 2021) If institutions work on hiring, training and retaining staff perhaps they can form nurse committees. These committees would work closely with the institutions to provide more training for nurses and other healthcare professionals. The training would need to focus on ethics and cultural diversity. The reason these topics should be the focus is because often the reason we fail at providing patient centered care is sometimes due to a lack of understanding and a lack of training. Especially with religious, social and cultural issues.
It is imperative that as healthcare professionals, we open our minds and make it a priority to respect a patient’s beliefs. We have to be sure that any bias and judgment is non-existent. This can be challenging at times as especially in a case in which a patient’s value or belief may compromise medical care. Of course, as nurses, we will never know or even understand everything about a patient’s values, culture or beliefs. It is important that we make every effort to be respectful of those values, culture and beliefs while the patient is in our care.
